이번 연구결과에서는 손상의 진행을 예측하는데 보다 효과적일 수 있다는 결과가 발표되었다. 질병활성도가 높을수록 손상이 많이 생긴다는 장기간 종단연구 결과가 발표된 만큼 이제 강직성 척추염도 최소한 낮은 질병활성도를 유지할 수 있도록 목표지향적인 치료가 이루어져야 하며 이를 통해 손상을 막을 수 있을 것을 기대해볼 수 있다.

본 증례에서 초기에 높은 질병활성도 상태였으나 적절한 치료제 선택을 통하여 낮은 질병활성도에 도달하여 유지할 수 있었고 3년 이상 추적 관찰 시 손상의 진행을 보이고 있지 않아 질병활성도 조절을 통해 손상을 막는 것을 기대하고 있다. 적어도 낮은 질병활성도를 유지할 수 있도록 (ASDAS < 1.3) 적절한 목표지향적인 치료(treat to target)를 통해 강직성 척추염의 증상뿐 아니라 손상을 막기 위한 노력이 이루어져야 하겠다.

■ 고찰
최근 류마티스관절염에서 조기진단과 조기의 적절한 치료가 병의 경과에 중요한 영향을 미치고 골손상을 막을 수 있다는 것이 강조되며 그 중요성이 부각되고 있다. 이에 비해 강직성척추염(ankylosing spondylitis; AS)의 경우에는 아직 치료가 골손상을 막는다는 근거가 부족하여 이에 대한 기대는 상대적으로 높지 않았었다. 물론 비스테로이드항염제의 지속적인 투여가 골손상을 막는다는 보고도 있었고 특히 ESR 및 CRP가 높은 경우에 더욱 효과적으로 막는다는 보고가 있었기는 했지만, 정작 임상적 증상 개선에 효과적인 TNF 억제제가 골손상을 유의하게 막는다는 보고가 없었다. 다만 지금까지 이러한 연구들은 연구기간이 짧았고 이미 염증이 오래 지속된 후에는 적절한 치료가 이루어지더라도 골손상을 막을 수 없다는 보고도 있어 초기에 적절히 치료하면 개선을 가져올수도 있다는 기대감은 있었고 보다 장기간의 연구를 기다리고 있었다.

최근 드디어 장기 종단 연구를(longitudinal study) 통해 강직성척추염의 적절한 치료가 골손상을 막을 수 있는지에 대한 결과가 발표되었다. Outcome in AS International Study (OASIS) 코호트를 이용하여 총 184명의 강직성척추염 환자를 대상으로 12년간 경과관찰을 하며 매 2년마다 임상적 결과와 방사선적 진행을 평가였다. 연구 참여 시점의 질병활성도가 골손상의 진행과 연관성을 보여 AS Disease Activity Score(ASDAS), BASDAI, CRP, ESR, 환자의 전반적 평가 모두 유의하게 골손상의 진행과 연관성이 있었다. 종단 분석에도 ESR을 제외하고 질병활성도의 척도와 골손상이 유의한 연관성을 보였다. 또한 남성인 것과 유병기간이 긴 것이 질병활성도 척도와 효과변경(effect modification)을 보이고 있어 골손상이 더 일어나는 것으로 나타났다. 반면 HLA-B27, 약제의 종류(비스테로이드소염제 또는 항TNF제), 관절 외 증상과는 연관성이 없었다. 여러 질병활성도 척도 중 ASDAS가 가장 골손상의 진행을 잘 예측하고 있어 질병활성도 평가를 위해 기존에 널리 사용되고 있던 BASDAI 보다는 ASDAS를 평가하여 치료 계획을 수립하는 것이 골손상을 방지하는데 보다 도움이 될 수 있을 것으로 기대해 볼 수 있다. 하지만 질병활성도나 염증 정도 만으로 척추관절염 환자들의 골손상을 모두 설명할 수 는 없고 실제 질병활성도가 없는 경우에도 골손상이 진행되어 질병활성도가 없다 하더라도 류마티스관절염의 관해(remission)와는 달리 보아야 할 것이다. 그렇다면 질병활성도를 낮추는데 효과적인 항TNF제를 사용하여 질병활성도가 낮아지면 골손상을 막을 수 있을 것인가? 여기에 대해서는 아직 섣불리 결론을 내리기는 이르다. 현재까지의 연구들에서 항TNF제를 이용한 치료가 인대골극(syndesmophyte)의 형성을 막지 못한다는 보고들이 있었고 반면에 비스테로이드항염제를 이용한 연구에서 TNF와 무관한 기전으로 인대골극의 형성을 억제했다는 보고도 이었다. 이제 강직성척추염에서 질병활성도가 골손상과 연관성이 있다는 것이 밝혀진 만큼 어떤 치료가 가장 효과적으로 골손상을 막을 수 있을지 연구하는 좋은 임상연구들이 이루어져 강직성척추염 환자들의 골손상을 막을 수 있기를 기대해 본다.

Abstract
Objectives To analyse the long-term relationship between disease activity and radiographic damage in the spine in patients with ankylosing spondylitis (AS). Methods Patients from the Outcome in AS International Study (OASIS) were followed up for 12 years, with 2-yearly clinical and radiographic assessments. Two readers independently scored the X-rays according to the modified Stoke Ankylosing Spondylitis Spine Score (mSASSS). Disease activity measures include the Bath AS Disease Activity Index (BASDAI), AS Disease Activity Index

(ASDAS)-C-reactive protein (CRP), CRP, erythrocyte sedimentation rate (ESR), patient’s global assessment and spinal pain. The relationship between disease activity measures and radiographic damage was investigated using longitudinal, autoregressive models with 2-year time lags. Results 184 patients were included (70% males, 83% HLA-B27 positive, mean (SD) age 43 (12) years, 20 (12) years symptom duration). Disease activity measures were significantly longitudinally associated with radiographic progression. Neither medication nor the presence of extraarticular manifestations confounded this relationship. The models with ASDAS as disease activity measure fitted the data better than models with BASDAI, CRP or BASDAI +CRP. An increase of one ASDAS unit led to an increase of 0.72 mSASSS units/2 years. A ‘very high disease activity state’ (ie, ASDAS >3.5) compared with ‘inactive disease’ (ie, ASDAS <1.3) resulted in an additional 2-year progression of 2.31 mSASSS units. The effect of ASDAS on mSASSS was higher in males versus females (0.98 vs 0.06 mSASSS units per ASDAS unit) and in patients with <18 years vs ≥18 years symptom duration (0.84 vs 0.16 mSASSS units per ASDAS unit). Conclusions This is the first study showing that disease activity contributes longitudinally to radiographic progression in the spine in AS. This effect is more pronounced in men and in the earlier phases of the disease.
